4. Medical Certificates and Proof of Hospitalization

(1) During your stay

  • To apply, please consult your ward clerk.
  • To receive the documents, please visit your ward clerk’s office.
    Office hours : 8:00 - 16:30
  • Documents necessary to apply: A receipt slip issued by the clerk when you apply.
  • If you would like someone other than yourself to receive the certificate, you will need a proxy.
  • If you would like your certificates mailed, please inform a clerk.

(2) After being discharged from Kameda

  • To apply, please visit the reception desk on the 1st floor of the K-Tower.
  • If you would like someone other than yourself to apply for the certificate, you will need to apply for a proxy.
  • To receive the documents, please visit the cashier counter located on the 1st floor of the K-Tower.
  • If you would like someone other than yourself to receive the certificate, you will need a proxy.
* It may take up to 2 weeks to issue the required medical certificates. Regarding the contents and circumstances, please understand that this make take much longer.

(3) Applying for the documents via post

Enclose the documents stated below, and send them via registered mail to the given address.

  1. 1 .Designated form from the insurer
  2. 2 .Document expense ( If you need the documents to send to an insurance company, the expense will be 7,700 yen including tax. Please ask for details of other purposes)
  3. 3 .A document containing the following requirements (any format is acceptable)
  • Name of patient
  • Kameda Hospital ID number
  • The period (dates) you wish to be certified
  • Contact address during day time
  • If you have asked someone else to apply for the document, you will need to state the relation and proxy.
  1. 4 .A copy of Kameda’s hospital ID card or copy of identification card (such as your driving license)
  2. 5 .Proxy documents and a copy of the applicant’s identification ( for example a copy of their license)
  3. 6 .Self-addressed envelope with 244 yen of postage stamp attached
