1. Hospitalization Procedure

(1) Please visit the Reception desk on the 1^st floor of K-tower when you come to Kameda Medical Center on your first day of hospitalization. At that time, please bring the following materials.

Kameda ID Card ( Gray card)
Japanese National Insurance if you have one.
The pledge of Hospitalization
The Referral form
Nursing questionnaire (Please write your address and provide answers to questions.)
The informed consent form

Please bring No.3-No.6, if you have them beforehand.

(2) We will inform you of the time you can be in your room prior to your arrival. Please note, however, that sometimes the room & time to enter may be changed depending on ward availability.
