昨日の高橋先生の講義でも出てきましたが、PCV13のACIPの推奨が変更されました。2019年6月のACIPのmeetingで、
免疫不全状態ではない65歳以上の成人では、PCV13を全員に推奨するものではなく、患者との共有意思決定に基づいて
推奨する（based on shared clinical decision making）とされましたが、2019年11月22日のMMWRで正式に報告されました。尚、髄液漏、人工内耳、免疫不全（本文中に表がありますが、HIV、無脾症、骨髄腫、固形臓器移植などが含まれます）では接種が必要です。

Use of 13-Valent Pneumococcal Conjugate Vaccine and 23-Valent Pneumococcal Polysaccharide Vaccine Among Adults Aged ≧65 Years: Updated Recommendations of the Advisory Committee on Immunization Practices

原文は以下です。
PCV13 vaccination is no longer routinely recommended for all adults aged ≧65 years. Instead, shared clinical decision-making for PCV13 use is recommended for persons aged ≧65 years who do not have an immunocompromising condition, CSF leak, or cochlear implant and who have not previously received PCV13.

以下のpopulationは、PCV13を接種する利益が、より高いとされており、接種において考慮が必要です。

• Persons residing in nursing homes or other long- term care facilities
• Persons residing in settings with low pediatric PCV13 uptake
• Persons traveling to settings with no pediatric PCV13 program

ちなみにですが、同じ2019年6月のmeetingでは、PCV13以外にも以下の推奨が出ています（https://www.cdc.gov/vaccines/acip/recommendations.html）

【HPV（9価）】

27-45歳で、過去のワクチン接種が不十分な場合、キャッチアップはroutineには行わず、患者との共有意思決定に基づき推奨する、としています。
これも2019年8月16日のMMWRで報告されています。

ACIP Recommendations

キャッチアップに関しては以下のように表現してあります。
Because HPV acquisition generally occurs soon after first sexual activity, vaccine effectiveness will be lower in older age groups because of prior infections. Some previously exposed adults will have developed natural immunity already. Exposure to HPV decreases among older age groups. Evidence sug- gests that although HPV vaccination is safe for adults aged 27 through 45 years, population benefit would be minimal; nevertheless, some adults who are not adequately vaccinated might be at risk for new HPV infection and might benefit from vaccination in this age range.

Adults aged >26 years. Catch-up HPV vaccination is not recommended for all adults aged >26 years. Instead, shared clinical decision-making regarding HPV vaccination is recom- mended for some adults aged 27 through 45 years who are not adequately vaccinated. (Box). HPV vaccines are not licensed for use in adults aged >45 years.

これまでは、primary series（Bexseroであれば、1ヶ月間隔あけて2回接種）の後のboosterについては特に言及がなかったのですが、以下のようにboosterが追加されました

• primary seriesが終了後、1年後に接種
• その後は、リスクが続く限りは2-3年毎に接種

こちらは、まだ正式にガイドライン等で発表されてはいませんが、今後要チェックです。
